Who Is Eligible?

All Member Agencies of ACTA are eligible to apply for Group Benefits Insurance. Agency owners and their employees must work a minimum of 20 hours per week and all eligible employees must apply for coverage.

Basic Life Insurance

  • All staff – a flat $20,000

Dependent Life Insurance

  • $5,000 spouse
  • $2,500 each child

Accidental Death & Dismemberment

  • Same benefit as the Basic Life amount
  • Double the Life Benefit if death occurs due to an accident

Extended Health Care

  • Prescription drug re-imbursement level of 80%
  • No annual deductibles
  • 100% re-imbursement of all eligible expenses
  • Semi-private hospital accommodation
  • 30 days out-of-country emergency medical and travel assistance
  • Paramedical coverage (chiropractor, physiotherapy etc.) up to $15 per visit and an annual maximum of $300.
  • $1,000,000 lifetime maximum

Basic Dental Care

  • Basic coverage with re-imbursement option of 80%
  • No annual deductibles
  • Recall examinations once every 9 months
  • $1,000 benefit maximum per person per calendar year

Major Dental including Ortho

  • Available to clinics with five or more employees with basic dental coverage
  • Major restorative (crown & caps) with 50% re-imbursement
  • $1,000 annual maximum per person ($500 1st year maximum)
  • Orthodontia – 50% re-imbursement with a $1,000 lifetime maximum
  • For dependent children under age 18 only

Vision Care Coverage

  • Coverage for eye glasses and contact lenses
  • Re-imbursement level options of $80.00, $150.00, or $200.00 every 24 months

Long Term Disability Insurance

  • 66 2/3% of monthly income to a maximum benefit of $5,000
  • Benefit payable to age 65
  • 119 day waiting period

Optional Life

  • Amounts of up to $750,000 may be purchased by members and spouses
